For your convenience, we've made our Jones Learning Center application available to you online. However, if you'd prefer to download and print out the application form and the supporting documents, these are the files you'll need:

You can download all the forms you'll need in a single zip-file. You'll need a program such as WinZip to extract the individual forms from this file.

All forms are saved in the Adobe pdf format, so you'll also need Adobe's free Acrobat Reader installed on your machine to view the files. You can get the reader at http://www.adobe.com.

Send the completed forms along with your High School transcript, a previous psychological test report documenting the learning disability or AD/HD, and your $75 application fee to:
